Our Care Coordination team has over 100 combined years of experience in the health and social care sector and we truly believe we can remove the burden many people face. We would consider taking on the Health and Welfare Lasting Power of Attorney role if the situation warrants it.
Our role goes beyond just navigating the care system - we are their life line and family support service.
